Our providers Our providers deliver preventative medicine, diagnosis, treatment of acute and chronic disorders, flu shots and other immunizations, minor surgical procedures and overall planning for a healthy future. Our providers work closely with other Eagle specialists and specialty providers in the area to coordinate diagnostic testing, consults, treatment and managing the referral process.

Immunization Policy: At all Eagle Family Medicine sites, we follow the American Academy of Pediatrics guidelines for immunizations and feel strongly in immunizing our patients to protect infants and children. We are unable to accept patients uninterested in immunizations.

Telephone Triage: Whenever possible, we ask that calls requesting medical advice be made during office hours when our staff has access to your medical records. If an urgent problem arises and you cannot wait for the office to open, you may call our after-hours answering service. A nurse from the nurse call center will contact you and offer medical advice that has been approved by our physicians. If the need arises, they will contact our on-call provider. Eagle is charged for this service but currently does not pass that charge along to you, so we are requesting that you utilize this service for serious concerns only.

Insurance & Payment Information and Policy: Please have your insurance information and payment with you at the time of service. All fees associated with each visit such as co-pays, are due at the time the services are rendered. For families in which the parents are either separated or divorced, the parent bringing the child to the office is authorizing treatment and is, therefore, the parent responsible for co-payment or co-insurance on the date of service. We will not call or contact the other parent to obtain payment information. As of November 1, 2024, a fee of $25 will be charged same day cancellations and for missed appointments without at least a 24 hours’ notice.

Reminders – Repeated “No Shows” or Missed Appointments will result in a dismissal from the practice. If you arrive 10 or more minutes late for your appointment, you will be asked to reschedule.
