What to Expect at Your Visit
Plan to arrive about fifteen minutes early for a first appointment so our front desk can verify your insurance and get your paperwork started. Bring a photo ID, your insurance card, a list of every medication and supplement you take with dosages, and the name and phone number of any specialist you see regularly.
During the visit you will have time to describe what brought you in without being rushed. Your provider will review your history, perform any needed examination, and talk through next steps. If lab work, imaging, or a referral is appropriate, our staff coordinates it before you leave and follows up with results through the patient portal.

How do I schedule an appointment with this provider?

What should I bring to my first appointment?

Do you accept my insurance?
Eagle accepts most major insurance plans, and our insurance partners page lists current plans in detail. Coverage can change year to year, so we recommend confirming with our billing team before your first visit. We also offer private-pay options for patients without insurance.
How do I request my medical records?
Records requests are handled through our medical records page, which explains the process and required authorization. Most requests are completed within a few business days. If you are transferring to another practice, we can send records directly to your new provider once we have your written consent.
What if I need care after hours or on a weekend?
Eagle Walk-In Care is open Monday through Friday from 7:00 AM to 7:00 PM and weekends from 9:00 AM to 3:00 PM, with no appointment necessary. It is also open most holidays. For a true medical emergency, call 911 or go to the nearest emergency department rather than waiting for an appointment.
How do I get a prescription refill?
The fastest way is through the patient portal, where you can send a refill request directly to your provider’s office. You can also ask your pharmacy to send us the request. Allow two business days for routine refills, and keep in mind that some medications require a visit before they can be renewed.
